BACKGROUND: Chicago will be our second OOH billboard location. We will be taking over the Millennium Metra Station and have a presence on 34 different ad spaces. To engage passengers and passers-by and encourage people to book SkinPen treatments, we’re planning a Gift With Purchase (GWP), which will be a SkinPen branded Simple Modern tumbler, for the first 200 people to submit their treatment receipt. In the fine print, we would like to stipulate that you must show proof of payment for at least three treatments. We plan to socialize this GWP via social media (including paid social) and on the Chicago.SkinPen.com destination website. There will be a submission form on the website where people can submit their receipts and redeem their gift. We would also like to retain the customer’s information so that we can include them in further direct-to-consumer communications. 

LANGUAGE AND TERMS & CONDITIONS FOR GWP REDEMPTION: 

1. Eligibility: This promotion is open to individuals residing in the Chicago metropolitan area who have purchased three (3) SkinPen treatments from participating providers within this area.
2. Promotion Period: The promotion begins on August 5, 2024, and ends on September 29, 2024. All purchases must be made within this period to be eligible for the gift with purchase. Purchase receipt(s) need to be submitted before October 31, 2024. 
3. How to Redeem:
   1. Purchase a minimum of three (3) SkinPen treatments from a participating provider in the Chicago metropolitan area.
   2. Retain your receipt as proof of purchase.
   3. Submit your receipt through Chicago.SkinPen.com before October 31, 2024.
   4. Include your name, contact information, and the date of purchase in the submission form, along with a short survey about your SkinPen treatment experience. 
4. Gift Details: Eligible participants will receive a Simple Modern tumbler with the SkinPen logo on the front. The gift is subject to availability and may be substituted with a similar item of equal or greater value at our discretion.
5. Limitations:
   1. The promotion is limited to the first 200 participants who submit their receipts. Only the first 200 people to submit will be able to redeem the gift with purchase.
   2. One gift per customer, regardless of the number of treatments purchased beyond the required three (3) treatments.
   3. This promotion cannot be combined with any other offers, discounts, or promotions.
   4. Offer is valid while supplies last.
6. Verification: All submissions are subject to verification. Incomplete or illegible submissions will not be processed. Proof of submission does not constitute proof of receipt.
7. Processing Time: Please allow up to six weeks for verification and delivery of the gift.

   2. SkinPen reserves the right to cancel, suspend, or modify the promotion at any time without notice.
   3. SkinPen is not responsible for lost, delayed, misdirected, or undelivered submissions.
